We need proof of residence. A CURRENT PIECE OF MAIL that has YOUR name and address on it will be sufficient for W.A.R.M.
This application will be used by our office to provide food for your family. It is also used to determine eligibility for assistance with utilities, dental and/or prescriptions. Food boxes are available monthly to Wise, Jack and Montague Counties. ONLY Wise County, Texas residents are eligible to receive financial assistance in addition to a food box.
You may print this application and have it filled out the first time you come see us or wait and fill it out in our office.
